To ensure a clear understanding of our services and mutual responsibilities, all clients are required to review and sign the applicable agreement and waiver for their chosen service or workshop.
As a courtesy, we've provided these documents below for your advance review.
How to Sign Your Agreement:
Please note: You only need to complete this form for your first scheduled session. A new form is only required if the terms of the agreement or participating individuals change.
Upon booking, a link to the form of your applicable agreement will be included with your confirmation email.
For Virtual Services & Workshops: The form must be completed virtually.
For In-Home or In-Person Services: You may complete the virtual form in advance, or a hard copy will be provided for your signature prior to the start of your session.
