Advance care planning is more than just paperwork; it’s a profound act of love for yourself and your family and a way to ensure your voice is heard when it matters most. It has become my personal mission to help as many people as possible navigate this journey with clarity and peace of mind.
Because I want to remove every possible barrier to completing your end-of-life planning, I’m doing something unheard of... There are no paywalls, no required donations, no pop-ups demanding your email address before you can access the materials, and no "masterclass" that ultimately turns out to be a hidden sales pitch. Whether you are a fellow North Carolinian (for whom some of these materials are primarily geared) or joining from further away, please take what you need.
My only ask is that you respect the heart and time I've poured into these resources. Please do not copy and distribute these materials as your own work. Instead, I'd love for you to direct others here so they can engage with these tools in their own way.

Page-by-page guidance to prepare your Living Will and Healthcare Power of Attorney documents.
Clear definitions of key terminology used within the forms, ensuring you understand every detail.
Education to help you make informed decisions about life-prolonging treatments, body disposition, and more.
Awareness and information about the Advance Instruction for Mental Health Treatment form.
Guidance highlighting the importance of establishing a Do-Not-Resuscitate (DNR) order and Medical Orders for Scope of Treatment (MOST) form.
Facilitation of one family meeting by request, to help share your wishes with loved ones.
2. Self-Guided Exploration of Contemplative and Practical Tools

Five short, comprehensive booklets, filled with guiding questions, prompts, and practical information to help you explore and document your preferences for the following topics:
Comfort & Care Planning: Thoughtfully articulate your preferences for personal comfort and care during the end-of-life journey, exploring desired physical, emotional, and environmental comforts.
Vigil & Farewell Wishes: Pre-plan the details of your vigil, funeral, memorial, or celebration of life, ensuring your final wishes are honored and reducing the burden on loved ones.
Contemplative Reflection: Engage in deeper personal reflection and inner exploration to ensure your end-of-life choices truly align with your core values, beliefs, and desired legacy.
Ethical Will & Legacy Document: Share your life’s wisdom, stories, and messages of love with family and friends, ensuring your values and personal history are preserved and passed on.
Financial Peace of Mind: Gain a foundational understanding of key financial considerations related to end-of-life planning, including a basic overview of Wills, Trusts, and the Probate process.
While aspects of this package are designed for self-guided exploration, you are never alone. Should you desire to be guided through any of the self-exploration content, require more in-depth, direct help with follow-through, or need assistance with any specific aspect, please reach out anytime. Our mission is to provide personalized support tailored to your evolving needs, ensuring you feel completely confident and at peace with your comprehensive plan.
Commitment: Six (6) hours virtually or in-home allows time for us to review your self-guided exploration tools and complete the guided core experience. Our time together may include 1-3 sessions.
5 hours education, preparation, document completion, and review of self-guided tools.

Additional support (greater than 6 hours): Sliding scale of $50-$100 per hour

This offering is ideal for those whose needs extend beyond the scope of the standard offerings above. The scope, estimated hours, and reciprocity for this customized support will be determined after an initial consultation.
